GSF is a nonprofit organization that has been providing services in Oregon for over twenty years. Initial work began with individuals who had developmental disabilities. GSF continues to offer The Gold Standard in person-centered care management and fiduciary oversight.
Everything we do is guided by your unique set of circumstances. This begins with an initial consultation. Whether you are looking for services now, or just seeking information about how to approach challenges in the future, we’re here and ready to talk.
GSF CARE is our person-centered care management division, specializing in Case Management, Guardianship, and Power of Attorney services for Oregonians requiring professional representation.
Case Management is a self-tailored menu of services that enlists GSF expertise to assist with an array of concerns and is bound by a service agreement that specifies the scope and intensity of care management interventions ranging from temporary to long-term. Case Management services are often sought by seniors seeking advice, counsel or interventions on a limited engagement basis.
Medical Power of Attorney services are available for individuals requiring critical intervention in healthcare decisions, or for planning end-of-life preferences. We work closely with healthcare professionals, including hospitals, hospice, and private caregiving providers to assure you are treated with dignity, respect, and that your wishes are executed to their fullest. We strive to be your voice, not your substitute.
Guardianship is available for individuals who require the highest level of care management in decision making assistance. Our team will assist you with daily affairs, care oversight, advocacy, and benefits assistance with compassion and dedication, as guided by the National Guardianship Association’s model code of ethics and best practices standards.
Financial Power of Attorney services are available for individuals requiring assistance with bill pay, asset marshalling, and estate planning priorities. Financial Power of Attorney engagements are normally considered as an add-on to other services being sought.
We invite you to contact our office to schedule an initial consultation to discuss your needs, concerns, and solution options. GSF is one of the largest providers of these services in the state of Oregon, and we are happy to partner with your multi-disciplinary team of medical and fiduciary professionals, as well as your loved ones, to address gaps in care you may feel you have, or to offer a more all-inclusive package designed to meet your individual needs. GSF CARE is overseen by a team of seasoned, professionals; including Stephanie Johnson, Licensed Clinical Social Worker, Bill Wolbrecht, MS, MDiv and, Dottie Jacobs, Nationally Certified Guardian/Conservator/Case Manager.
